Abilis Health Plan includes coverage for your Part D prescription drug plans. Your Nurse Practitioner will review your medications upon enrollment to ensure you have the right medications for your individualized care plan.
Each Abilis Health member works with a Nurse Practitioner whose proactive focus is on wellness care. Membership starts with the completion of a thorough Health Risk Assessment. The results are used to create a personalized care plan in coordination with the on site Primary Care Physician and the facility/community care team. The result is meaningful patient education, comprehensive clinical services and reduced hospitalizations.
FI-SNP is short for Facility Institutional Special Needs Plan. IE-SNP is short for Institutional Equivalent Special Needs Plan. Abilis Health plan is designed to meet the needs of residents through its unique model of care. You must be a resident of a long term care facility or community to be a member of this plan. You must also have Medicare Part A & B.
