2026 MODEL OF CARE TRAINING ATTESTATION
MANDATORY REQUIREMENT
As part of required CMS mandated annual training, Abilis Health (formerly Signature Advantage) has developed the Model of Care program for Abilis’ Institutionalized Special Needs Plan (ISNP) members. The Model of Care program serves as the foundation for our care management policy, procedures, and operational systems.
What Providers Need to Do
- Complete the training located on the Provider Documents page of the For Providers section of our website.
- Complete this online attestation form. If it is a group training, please upload the attendance roster of all team members who attended the training.
This Attestation and attendance roster will serve as evidence of completion for Abilis’ Model of Care Provider training.
